Well, that's embarrassing... I just realised that the version of Smoke2EXR available from the link was actually the wrong version (1.10 instead of 1.11). However, I realised this by also realising that the 1.11 version didn't actually work with Blender version 2.79 due to an issue with 'pack' (at 2.80 this no longer needs to be from a 'saved' image - at 2.79, it does). I've now updated the version to 1.13 to include the fix to that problem as well as being able to build as a grid of images - rather than a single row (which could cause precision errors with a large smoke domain).
